How do I get copies of my medical records?
There are two ways to obtain your medical records. You may call Health Information Management at 256.301.3380 and follow the instructions, or you may visit our office in the hospital. If you visit you will need to bring your government-issued photo identification and sign a release. You will then receive your records within 48 hours, excluding weekends and holidays.
